Form Questions
In the second step, you can select the form questions. Currently, you can choose from pre-populated form questions: Name, Phone Number, Email Address, City, Zip Code, State, and Country. Additionally, you can also ask for company name, job title, email address, and work phone number. Qualifying questions are also available for specific industries. For example, automotive companies can ask, "What type of vehicle are you interested in?" In this step, you are also asked to give your form a headline and description and provide a link to your privacy policy.

In this step, you can create the thank you message that will be Self Employed Database displayed after a lead completes the form. Use the post-submission headline and description to manage expectations and provide information about what the lead should expect next.

4. Choose How You Want to Receive Leads
Finally, you'll be asked to select how you want to receive your leads.
